Dear Church Member, to make your photography experience easier, we have a
form that can be filled out before hand. After filling out all of the red
fields, please print and bring with you on the day of your photography
session. Have Fun ! You need Adobe Acrobat Reader to view and print the following form. If you don't have Adobe Acrobat Reader installed, or you can't see the form, then please click on the following Adobe Acrobat Reader link. Click on the run button, and then click the run button again. This will start the install process of Adobe Acrobat Reader.
